Four Steps to Use chdpgateway.com

Step 1: Register

• Log onto www.chdpgateway.com, enter your contact information and click on Submit
• Once you complete the registration process, you will receive notification that the chdpgateway.com registration has been successfully completed

Step 2: Log onto chdpgateway.com and enter new user ID and password from notification

• Your contact information will be automatically pre-populated onto each new PM-160 Information Only form

Step 3: Enter the member’s demographic and CHDP assessment data.

• Complete all required fields. If these are not completed, the e-portal will not allow the form to be submitted.
• Once a member’s demographic information is entered into chdpgateway.com, it is automatically pre-populated onto each future PM-160 Information Only form for that member.

Step 4: Submit the PM-160 Information Only form.

• Once submitted, continue with additional PM-160 Information Only forms; you do not need to log out
